MEDICAL DIRECTOR

DR. WENDI LUNDQUIST, DO FAAPMR

Dr. Lundquist is the Medical Director and founder of Active Life Physical Medicine & Pain Center, Innovative Surgery Center, and Regena Spa. She is dual board certified in Physical Medicine and Rehabilitation along with Pain Management and a diplomat with the American Board of Pain Medicine. She completed her training at Loyola University in Chicago, Illinois in July of 2005. During her time there, she served as Chief Resident and was involved in several research projects. Prior to, she completed medical school at Midwestern University at the Arizona Collage of Osteopathic Medicine campus in Glendale, Arizona and internship at Doctors Hospital in Massillon, Ohio. She also has a Bachelor of Science in Biology and minor in Chemistry from the University of New Mexico in Albuquerque. She has great enthusiasm towards regenerative medicines and a special interest in sports medicine and pain.

I am devoted to providing you the highest quality medical care and interventions to ease your pain and promote an active life. As Sir Isaac Newton said, “A body at rest tends to stay at rest, and a body in motion tents to remain in motion.” This applies to patients who endure pain. Remaining active if paramount.

DR. DANIEL T. SMITH, MD FAEM

Active Life Physical Medicine & Pain Center continues to move upward and onward with holistic treatments. Dr. Daniel T. Smith joined Active life in September 2015 after his 24 year career in the United States Air Force attaining the rank of Colonel, and service as Deputy Group Commander of the 56th Medical Group, Luke Air Force Base, AZ. He also served as a Squadron Commander, Chief of Aerospace Medicine, Public Health Emergency Officer and as team lead for Critical Care Air Transport with deployments to Bahrain, Saudi Arabia, Kandajar, Afghanistan, and United Arab Emirates. He has many years experience as a board certified Emergency Medicine physician and certified Acupuncturist.

He received his commission to the Air Force upon entering the Uniformed Services University of Health Sciences Medical School in 1991 and graduated with a doctor of medicine in 1995. After service as Flight Surgeon, he completed his Emergency Medicine Residency in 2001 at Wright State University, Dayton, Ohio and later attended the HMI Medical Acupuncture for Physicians SMART course in 2009. He has extensive experience with acupuncture in a deployed setting as well as in the clinic. He is excited to join the Active Life team and brings a wealth of experience, knowledge, and compassion and success to the fight against chronic pain.

He enjoys spending time with his beautiful family including his wife, Anne and 7 children. He is involved in nutrition and exercise, and serves actively in the community and in his church.

MAEGAN KITZMANN, MMS, PA-C

Maegan is a recent graduate of Midwestern University’s Physician Assistant Studies program. Throughout her time at Midwestern, she developed a strong interest in physical medicine and rehabilitation, sports medicine and orthopedics. She rotated with Dr. Lundquist and Dr. Smith as well as the head physician for the San Diego Padres. Prior to Midwestern University, she completed her Bachelor of Arts and Psychology from San Diego State University and went on to complete her Masters of Arts in Clinical Psychology with an emphasis in Marriage and Family Therapy from Pepperdine University. She is looking forward to using all of the skills that she has learned throughout her education and former practice as a Marriage and Family Therapist Intern in treating patients’ bodies and minds in order to heal. She is originally from Southern California and enjoys sports, movies, music and spending time with her family, friends, and fur baby.

OLIVIA NGUYEN, MMS, PA-C

Olivia is Active Life’s new on-boarding physician assistant and is a recent graduate of Midwestern University’s Physician Assistant Studies program. During her studies at Midwestern, she developed an interest in physical medicine & rehabilitation and orthopedics, prompting her to complete a rotation at Active Life,where she loved getting to know the staff and patients. While rotating in orthopedics at Mayo Clinic, she gained experience in assisting in joint replacement surgeries including knees, shoulders, and hips. During her time at Midwestern, she enjoyed serving as a H.O.M.E. medical clinic coordinator at the Vista Colina shelter and was rewarded the Brent T. Wright Scholarship from her PA program. Before working on her masters, she received a Bachelor of Science in Biological Science and a history minor from University of California, Santa Barbara. She is originally from Southern California and enjoys music, tennis, and swimming.
